We also have some books that may assist you.  I am attaching froms that may or may not be what you need.  There are a lot of variations in forms.  Those are sample forms that are intended to be filled out on line.  To search Legally Yours away from the library you will need to sign in with your library card and pin.  We also have books in the library as can be seen in the link that will may assist you with getting the will or the power of attorney form that is just right for you.
